Carlyle CUSD #1 district-wide registration will be on Tuesday, August 1st from 8:00 a.m.-7:00 p.m. Our district-wide registration will take place in the High School Library. 

Just as a reminder the Health Requirements are as follows:

• Kindergarten: Physical and updated shot record, showing proof of booster doses for Dtap, polio, MMR, and varicella 

• 6th grade: Physical and updated shot record, showing proof of Tdap and meningococcal vaccines 

• 9th grade: Physical and updated shot record, showing proof of 2 varicella vaccines

• 12th grade: Proof of 2 meningococcal vaccines (only 1 required if first dose received at 16 years old or older)

• Eye exams: Kindergarten requirement; due October 15, 2023

• Dental exams: Kindergarten, 2nd, 6th, and 9th grade requirement; due May 15, 2024
